Our aim is to use the next several years to make the biggest national move to integrated care of any major western country. This will take the form of Sustainability and Transformation Partnerships covering every area of England

Sustainability and Transformation Partnerships (STPs) are responsible for planning and delivering care in England.
STPs were established to improve health and care services; to make sure that services are integrated, cost efficient and, most importantly, that they meet the needs of the local population.
Currently, there are 44 STPs and they are made up of the following:
- Health sector organisations
- Local authorities
- Third sector organisations
- Community partners
- Individuals.
Their first task was to develop STP plans for the next five years; up to year 2020 / 2021.

CNWL has taken an active part in STPs within the three localities where we are based:
