Community Services Month is in full swing, our first week focused on unplanned care and we've heard from staff across the Trust and held events in-person and virtually.
Unplanned care teams do such a fantastic job supporting people who need urgent care and delivering that care in their homes within two hours of referral. Without their interventions people would end up being admitted to hospital but instead can be cared for in their own homes.
Fun fact, our London Urgent Community Response team is one of the largest in the country, covering six boroughs.

Milton Keynes Community Services visit Shenley Wood Retirement Village
MK community services visited Shenley Wood Retirement Village to promote our local and specialist services with staff and residents.
Lorretta Carson, Specialist Continence Nurse as well as Amanda Black and Louise Addis from the Home First Frailty Team attended.

Preparing for Winter
At CNWL, we begin our winter planning with partner organisations during the summer, leveraging insights from past experiences to forecast pressure points and develop strategies to care for our community.
CIS Carer Champion in unplanned care
The team's carer champion work is progressing and they are working with the Trust's Patient Involvement Team with the aim of completing and achieving the triangle of care self assessment by the April 2025.
Physical and Mental Health Integration in Hillingdon
A large number of our elderly population receive care from both our physical health and mental health teams. In Hillingdon, CNWL teams are working on ensuring pathways are integrated.

Next week we turn our attention to starting well, where we will shine a light on CNWL services that support our newest members of the community and give children and young people the best possible start in life.
