Empowering patients to manage their diabetes effectively is a core focus of the Hillingdon Diabetes Team. We offer a range of education sessions for those newly diagnosed, including a monthly carbohydrate awareness session to guide lifestyle and dietary changes.
A dedicated support clinic is available on Tuesday mornings for patients and carers to call or email for advice on medication adjustments.
Our use of digital technology further promotes self-management, making it easier for patients to engage in their care. Additionally, we have introduced a two-step appointment system: the first appointment is with a diabetes technician who educates patients on self-monitoring and management, ensuring they are well-prepared for their follow-up appointment with a clinician.
