Focus on Milton Keynes Urgent Community Response Team

Our fabulous team in Milton Keynes are doing lots of work to ensure their team is contacted at the right time so they can support as many people as possible.  They are going live with Consultant Connect which will bring many benefits including call recording.  Additionally, they are launching a ‘call before convey’ pilot with two care homes that have the highest conveyance rates to A&E.  Staff in the homes will contact our team before calling 999 during the pilot. 

Silver Triage

Many older people living with frailty are taken to hospital by ambulance but then turn out not to need hospital care. The Silver Triage initiative in North Central London aims to make sure more older people living with frailty get urgent care in their home setting.  The initiative enables specialist geriatricians (a medical professional that cares for older people) to provide advice to ambulance paramedics.  The scheme has:

  • Seen a substantial reduction in patients taken to hospital, from 75 to 20%
  • 100% of paramedics said they would use the system again

Intermediate Community Bed Escalation (ICE) Hub

The ICE Hub supports patients from hospital into community rehabilitation beds through:

  • Single waiting list for community-based rehabilitation beds
  • Reduces duplication of referrals to multiple units at once
  • Shorter delay times if patents need to be diverted to another unit
  • Faster screening as notes passed between units through ICE
  • Opportunity to coordinate response to acute pressures, and better understand demand and capacity

CNWL Frailty Lab

Based at St Pancras Hospital, this helps improve health outcomes for older people.  It is a purpose built facility which includes a clinical assessment area, as well as dedicated resources and equipment.

The lab is a collaboration between CNWL and University College London, led by Professor’s Simon Conroy, Daniel Davis and Alex Tsui, aiming to improve the assessments and treatments for older people.  Find out more here: Keeping people healthier as they live longer: Healthy Ageing Lab opens at St Pancras Hospital :: Central and North West London NHS Foundation Trust
