From 1 July 2024, community services delivered by CNWL in Camden will be working more closely with our partners in the Council, social care, primary care and the North London Mental Health Partnership (formerly Camden and Islington NHS Foundation Trust).

This will be known as a ‘neighbourhood’ model; and we hope that integrating these services more will mean a better experience for our staff and the residents who use our services.

What are we doing?

Neighbourhoods will bring together CNWL’s community adult services in Camden (known as Camden Integrated Community Healthcare) with our partners who will work together as new multidisciplinary teams.

This means our services will move into five new neighbourhood teams:

  • Central Camden Integrated Community Healthcare
  • East Camden Integrated Community Healthcare
  • West Camden Integrated Community Healthcare
  • North Camden Integrated Community Healthcare
  • South Camden Integrated Community Healthcare

Why are we doing it?

Ultimately, we want to create better outcomes for residents and more empowering working environments for our staff in Camden.

This new model will help build relationships across teams and organisations, and it means that we spend less time trying to find the right person to speak to so residents feel less lost in a complex system of services. It also improves our ability for joint care and holistic support. 

Will patients see any changes in the care and support they receive?

We will continue to provide nursing and rehabilitation care for people in their own homes and other community settings. Patients, service users and carers will see no difference in the NHS care and support that they receive.

The changes to our teams mean that patients might be allocated a different nurse or therapist. Your current clinician will advise you of any changes in advance.

We’ll be writing to patients as well to make sure you know the new contact details if you need to speak to the team.

We will continue to accept referrals from GPs and other health and social care professionals, voluntary sector workers, patients, carers and families.
