National Diabetes Awareness Day, marks the birthday of Sir Frederick Banting who co-discovered insulin along with Charles Best in 1922. 

As part of Community Services Month, we’re spotlighting our Diabetes Team in Hillingdon that supports nearly 3,000 individuals who are living with diabetes, often alongside other complex long-term health conditions.

Hillingdon’s multidisciplinary team is composed of diabetes specialist clinicians, allied health professionals (e.g. physio therapist, podiatrists), doctors, and nurses. Together, they deliver face-to-face clinics throughout the Hillingdon and offer virtual consultations on a weekly basis.

The service runs a dedicated weekly insulin titration clinic, which, combined with an email support service, assists over 100 patients each week. This initiative supports the safety of individuals who are either new to insulin therapy or managing multiple health complexities.

The team provides in-home care for patients with mobility challenges. To further extend our support, we have a clinician available daily through our Single Point of Contact (SPOC) service, where healthcare professionals across Hillingdon can call a hotline for guidance on managing patients’ diabetes medication issues.

A weekly multidisciplinary team (MDT) meeting is also held to discuss patient cases, ensuring the highest standard of care is consistently delivered.

Training sessions are held monthly for all in the team, to keep clinicians’ skills and knowledge up to date and ensure they are always working to best practice standards.
