CNWL’s Hillingdon Transition Service has published two new leaflets. The Transition Service form part of the Hillingdon Community Children's Nursing Team.
The first leaflet provides information about the Hillingdon Transition Service, who work with children and young people, their carers and families with the transition from children to adult services. It also includes advice for young people on maintaining their health and wellbeing.
The second leaflet includes information about the transition to adulthood for young people with learning disabilities and complex needs.
