Staff across the Trust hosted the first lunch and learn for Community Services Month  which focused on unplanned care and the work of our Urgent Community Response teams. 

Our Urgent Community Response team is one of the large in the country, covering six boroughs and see people in the community experiencing a health crisis within two hours of referral.  Without their intervention people would end up being admitted to hospital but instead can be cared for in their own homes. We heard from:

  • Hannah who is working in Milton Keynes with South Central Ambulance Service to push patients from their call centres to our teams
  • Keeley about work they have done in Westminster and Kensington & Chelsea to reduce falls in Care Homes.  They have seen a 51% reduction in one Care Home and are implementing across the whole boroughs now. 
  • Alastair who is looking to put in place an enhanced pathway for patients experiencing delirium so they can be cared for in the community
  • Gareth who is leading digital innovation work around DocAbode.  This HSJ shortlisted initiative is seeing substantial increases in the number of clinical visits staff can deliver in a day, whilst improving the safety of our staff.  You can see more here - Urgent Community Response – Dynamic Scheduling | Doc Abode
  • Ellie on how the Milton Keynes Virtual Ward they have setup has now seen 1,347 patients with an average length of stay of 9.6 days, resulting in over £300,000 of savings to the wider system
