Hillingdon Community Cardiac Rehabilitation Service helps the recovery of adults who have suffered an acute cardiac event or who have a chronic cardiac condition.
The service sees any patient over the age of 18 registered with a Hillingdon general practitioner (GP) and who has suffered an acute event or chronic cardiac condition.
The service accepts referrals from GPs, healthcare professionals, and self-referrals from patients who have had a recent cardiac event.
Referrals should be sent by fax to the service on 01895 484810.

The service operates Monday, Tuesday and Wednesday, by appointment only.
The service makes domiciliary visits by prior arrangement at the patient’s home address.
