Milton Keynes District Nursing Team supports housebound patients by providing nursing care in their own homes and/or other community settings.
The service is for patients aged 18 and over who are housebound or have a condition or procedure which can only be carried out in the home and have a nursing need. Patients must be registered with a Milton Keynes general practitioner (GP).
The service accepts referrals from individuals and healthcare professionals direct to the team or via the patient’s GP. Once we have received your referral we will see if our service best fits your needs.
If we do not feel we are the best service to meet your needs, we will advise you of a more appropriate service.
