Milton Keynes Early Stroke Rehabilitation Team provides rehabilitation for stroke survivors in their home.
We provide rehabilitation for stroke survivors in your home. Our aim is to help you regain as much independence as possible following a stroke. Together we will work to support you to achieve your goals. We can help with increasing your independence in activities such as washing, dressing, shopping, cooking and leisure activities.
We also help with:
- Adjusting to the emotional consequences of the stroke for you and those around you.
- Help with your mobility.
- Finding the best way to communicate if you have speech difficulties.
- Finding ways to cope with any changes to your thinking or memory.
- Help with eating and drinking safely.
The team comprises of a team coordinator, nurse, occupational therapists, speech and language therapists, physiotherapists, rehabilitation assistants and a psychologist. If you need any more help at the end of 6 weeks, you will be referred, if appropriate, to another service that may be able to assist you.
Individuals see the team for varying amounts of time. This may be a one-off visit or regular visits over a period of up to six weeks after hospital discharge, depending on their needs.
The service is available to people over the age of 18 who have had a new stroke and are registered with a Milton Keynes general practitioner (GP)
Individuals are referred into the service by their hospital consultant or GP.
The service is available Monday to Friday 9am to 5pm excluding Bank Holidays.
