From 1 April 2025, CNWL’s Staying Steady team will deliver the falls prevention service in Milton Keynes, previously provided in partnership with B-Well.

Staying Steady Milton Keynes offers specialist support to individuals, mainly aged over 65 years, who have fallen or are at risk of falling. 

We work with an individual and their carer to make decisions about the best way to reduce the risk of the person falling, raise awareness of risk factors for falling and make any necessary changes to their home to prevent falls.

We are part of the Milton Keynes Intermediate Care Service and Home 1st. We work in partnership with social services and other primary care services such as district nurses, general practitioners (GPs), South Central Ambulance Service, Community Alarm Service, Age UK Milton Keynes and Milton Keynes University Hospital NHS Foundation Trust. 

The team includes physiotherapists, occupational therapists, nurses, therapy assistants, management and administrative staff.

The service is provided to any person (primarily those aged over 65 years of age) registered with a Milton Keynes Clinical Commissioning Group general practitioner (GP) who has fallen or is at risk of falling.

The service accepts referrals from health and social care professionals. The service also accepts self-referrals from people who have fallen, or their family members.

All referrals are received via our Single Point of Access (SPA). Referrals can be made to the single point of access telephone 01908 801 108 or email cnwl.mkchsadultspa@nhs.net

Once a referral is received, it is triaged and the most appropriate pathway is sought based upon each individuals need; this can include onward referral to the voluntary sector, a home environment assessment, attendance at an exercise class or completion of a specialist Multifactorial Falls Risk Assessment (MRFA).

The service is available from 8:30am to 4:30pm, Monday to Friday (excluding bank holidays).

Our service is community based, therefore we do not have a location for patients to come to us. If patients have been invited to attend a clinic appointment or exercise class they will be advised of the location and directions as required.
