Neuro Navigators support people with complex neurological conditions to find the best course of rehabilitation.
They work with patient's, their families and clinicians to ‘navigate’ their rehabilitation journey, helping them move between hospitals, rehabilitation units and home or care homes as smoothly as possible.
This service is for adults with a neurological condition aged 18 or over who live within the outer North West London boroughs.
The individual can be referred at any point along their rehabilitation journey for support accessing appropriate rehabilitation services.
The Outer North West London Neuro Navigation service covers the boroughs of:
- Hillingdon
- Ealing
- Harrow
- Brent
- Hounslow
Neuro Navigators can visit patients in hospital or rehabilitation settings, in their own homes or in care homes as required.
The Neuro Navigation Service welcomes referrals from health and social care practitioners and voluntary agencies.
