The Hillingdon Primary Care Mental Health Team (PCMHT) work with GPs to help people better understand their mental health and wellbeing . Our team will support you to live as healthily and independently as possible. We will work with you for a set number of sessions to focus on the particular difficulties you may have.
All adults aged 18 years and over who are registered with a Hillingdon GP
People registered with a GP from the Concorde, Wellcare and Clover Networks, can directly refer to the service. You will then see a worker from the PCMHT. The appointments will be at the surgery and the allocated worker will work closely with the GP.
People registered with a GP from the Metro Health Network and have been discharged from the Community Mental Health Team (CMHT) in CNWL, can be referred back to GP care with the support of the PCMHT. Referral would be made by the CMHT and appointments would be at the GP surgery.
People can also be referred from the CMHT who have reached a stage in their recovery where no they longer require CMHT services. We will offer limited time supportive interventions to assist in the transition.
Hours of operation are Monday to Friday, 9am until 5pm. Clinic times at the GP surgery will be available from the GP reception.
