The Milton Keynes Community Mental Health Hub brings together a wide range of mental health professionals working together as a multi-disciplinary team who can provide evidence-based health and social care interventions. With service users at its heart, our aim is to deliver excellent care at the right time with a mutual focus on recovery.
The role of the team includes:
- Assessing, treating and reviewing symptoms.
- Taking optimistic views about recovery-focused interventions.
- Providing a wide range of psycho-social interventions and support.
- Providing support and intervention for family and carers.
- Assessing social care needs and providing social care services through personalised budgets.
- Working in partnership with a range of statutory and non-statutory services.
- Working closely with local GPs to provide joined-up mental and physical healthcare
- Adults over the age of 18.
- Are a resident of Milton Keynes or registered with a general practitioner (GP) in Milton Keynes.
- People with enduring or long-standing mental health issues.

Referrals can be made by a GP, social services or other health professionals to our Single Point of Access (SPA) cnw-tr.spa@nhs.net. We regret we are not able to accept self-referrals.
Once your referral is received, it will be screened by a mental health professional who may need to contact you or the person who referred you to gather more information. It is possible that following this you may be given details of organisations we feel are more appropriate to meet your needs.
If secondary mental health services appear to be the right place for you, you will receive an appointment for assessment with one of our practitioners. Our aim is to assess you within 28 days of your referral, though we are conscious that some people may need to be seen earlier and there will be provision for us to support more urgent assessments if necessary within 7–14 days.
